Creating and Managing Case Facts

Case Facts are key pieces of information extracted from documents that are essential for understanding and managing a case. These facts include critical details such as dates, decisions, medical evaluations, conditions, claims, parties involved, and other relevant data points that provide a clear and comprehensive view of the case.

In CaseScribe, Case Facts can include:

  • Fact: The main piece of information or event relevant to the case.

  • Page Number: The specific page in the document where the fact is found.

  • Date: The date associated with the fact, providing a timeline of events.

  • Tags: Tags are applied to facts to make it easier to group themes like conditions or actions required.

  • Comments: Notes or annotations added to a fact by users in your organization.

  • Key Fact: A fact marked as particularly important to the case.

Fact Extraction of Documents

After classifying documents into their correct types and classifications, go to the Documents tab and review those in the Documents with Missing Facts category that the system identified as Unable to extract facts. Please process manually.

Open each document and determine why facts were not extracted. Common reasons include:

  • The document does not contain any relevant information.

  • The document is blank.

  • The document is misclassified.

  • The document is not editable, is an image, or has other factors preventing the system from extracting information.

Not all document types require fact extraction. If a document does not contain information that needs to be indexed as a fact, you may leave it as it is.

Fact Extraction Verification

If a document does not contain any relevant information to be indexed, you may leave it as it is.

Some documents require fact extraction, but the system may fail to do so. If a document contains important information but facts were not extracted on the first attempt, try extracting again:

  1. Click Restart AI on the upper right corner of the bucket that needs to be processed again.

  2. Sometimes the system may successfully extract text on the second attempt, but this is not guaranteed.

Always review AI-extracted facts to make sure important information, dates, and details are accurate.

How to Add and Edit Facts

If the AI fails to extract a fact from a document, you can manually enter the necessary information:

  1. Open the document.

  2. Click Add Fact.

  3. Fill in the fields for Fact Text, Page Number, and Fact Date.

  4. Add tags when applicable.

  5. Save the fact.

Depending on your organization's settings, CaseScribe may automatically apply tags to extracted or manually created facts. Organization administrators can control whether automatic tagging is enabled and whether CaseScribe applies Condition Tags, Generic Tags, or both.

Adding Multiple Entries of Facts from a Single Document

You can add multiple entries of facts separately from the same document. Follow these steps:

  1. Open the document.

  2. To add a separate fact:

    • Option 1: Highlight the relevant information in the document, then click Add Fact.

    • Option 2: Click Add Fact in the document reader, then fill in the necessary information.

You can also create facts while reviewing a document in Super Reader.

Creating and Tagging Facts Using ASK AI

ASK AI can also help create and organize facts while you review a case.

After ASK AI identifies relevant information, you can ask it to:

  • Create a fact from the information it found.

  • Apply a tag to referenced facts.

  • Create a new tag if the requested tag does not already exist.

For example, after ASK AI identifies relevant information, you can ask it to create a fact and apply a specific tag.

Facts created through ASK AI are identified separately from facts created through document extraction or manually by a user.

Revising Facts Using the Facts Tab

You can double-check and edit facts generated by the system and users directly in the Facts Tab.

You can edit the following fields to reflect the correct information:

  • Fact Date

  • Fact

  • Tags

  • Comments

Note: You can add, edit, and delete facts from both the Facts Tab and Documents Tab.

The Facts toolbar also provides quick access to commonly used tools, including:

  • Filters

  • Key Facts

  • Expand Rows

  • Group by Document, Date, or Tag

  • Export

Note: You can add, edit, and delete facts both in Facts Tab and Documents Tab.

Key Facts

Key Facts allow you to identify facts that are especially important to the case.

You can use Key Facts in the Facts Tab to focus your review on the most important information without removing the remaining facts from the case.

When Key Facts Only is enabled, CaseScribe displays a banner showing how many Key Facts you are viewing. You can select Show All to return to all facts.

Depending on your organization's settings, CaseScribe may automatically identify Key Facts during fact extraction. Organization administrators can disable automatic Key Fact assignment if the firm prefers to select Key Facts manually.

Identifying Who Created a Fact

CaseScribe tracks how facts were created. Using the Fact Creator filter in the Facts Tab, you can filter facts by:

  • System: Facts created during document extraction.

  • Ask AI: Facts created through ASK AI.

  • Individual users: Facts created manually by a specific user.

This makes it easier to review the source of facts and distinguish between automatically extracted, AI-created, and manually created information.

Fact Comments

Comments can be added to facts to leave notes, questions, or other information for your team.

ASK AI can recognize when facts contain comments and may read those comments when they are relevant to your question.

Comments are treated as annotations rather than evidence. ASK AI does not cite comments as facts or count them as supporting evidence. Comment authorship is also preserved so ASK AI can distinguish between comments added by different team members.

Grouped Facts

CaseScribe can automatically group duplicate or overlapping facts into a consolidated fact to reduce repetition in large cases.

Grouped facts preserve the connection to the original facts and supporting documents. If an individual fact in a group is marked as a Key Fact, the grouped fact also inherits the Key Fact status.

You can use the Grouped Facts filter in the Facts Tab to review grouped or individual facts.

Fact Grouping may depend on your organization's settings. Organization administrators can enable or disable Fact Grouping for the organization.

For more information about how grouped facts work, see Fact Grouping in CaseScribe.

Commonly Asked Question

What is the difference between a fact date and a document date?

Document Date: The date when the document itself was created, signed, or received. In CaseScribe, it is the date noted by the VA in the filename of the document.

Fact Date: The specific date associated with the event or piece of information extracted from the document. Some documents may have similar dates for the fact date and document date.
