This release includes a series of targeted improvements focused on enhancing fact extraction across VAMC documents, C&P examinations, Independent Medical Evaluations (IMEs), and Service Records. These updates are designed to improve accuracy, reduce manual review, and streamline case development.
VA Medical Records (VAMC) Fact Extraction Enhancements
We’ve significantly upgraded how CaseScribe handles large-scale VA medical records particularly CAPRI files of up to 3,000 pages. This enhancement is designed to help attorneys focus only on legally material evidence, avoiding irrelevant content while ensuring that high-impact details are never overlooked.
Key Improvements Over Previous Extraction Logic:
Legal-First Prioritization
Evidence is now scored by adjudicative relevance, not medical breadth. Each fact receives a priority score based on its potential legal impact in a disability claim.
Strict Fact Consolidation
Related entries from the same day or care episode are now grouped. Redundant notes, repeats, and minor variants are automatically consolidated to reduce clutter.
Hard Ignore List Implementation
Routine or low-value entries—such as admin notes, templated text, raw lab data, medication refills, scheduling, and education materials—are now excluded by default.
Functional Impact Emphasis
The system now captures clear, actionable language related to functional limitations. This includes verbatim quotes related to physical capacity, activity duration or distance limits, and causal statements linking symptoms to service.
Enhanced C&P / DBQ Fact Extraction
We’ve refined our extraction capabilities for Compensation & Pension (C&P) exams and Disability Benefits Questionnaires (DBQs) to better capture key clinical details:
Laterality Recognition
The system now consistently identifies and records whether a condition applies to the left, right, or both sides—particularly for orthopedic issues such as knee, wrist, or ankle conditions.Tinnitus Diagnosis Detection
Tinnitus is now reliably captured when documented in medical records, addressing previous cases where it was inadvertently omitted.Activities of Daily Living (ADLs) and Functional Impairment
Extraction now includes use of assistive devices, notes from Aid & Attendance reports, and content from the “Functional Impact” sections of C&P exams.Symptom and Examination Details
Narrative descriptions of symptoms and physical exam findings are now captured more reliably, providing greater context for each condition.

Assignment and Deployment History
Records now include detailed listings of duty stations, base assignments, and overseas service—along with assignment codes and abbreviations where available.Performance Reports and Duty Descriptions
Additional content is extracted from military personnel records to support exposure identification (e.g., potential contact with burn pits, airborne hazards, or toxic substances).Refined DD214 Identification
The system more accurately distinguishes DD214 forms within military personnel records, reducing false positives where non-DD214 documents were previously misclassified.Expanded STR Coverage
New rules enable the extraction of both the Report of Medical History and the Report of Medical Examination from Service Treatment Records, even when embedded in longer files.
Improved Extraction of IME Conclusions
We’ve expanded our extraction rules for Independent Medical Evaluations (IMEs), including those within Private Medical Records (PMRs):
Medical Expert Opinions
The system now identifies and extracts the final conclusions or nexus opinions offered by the evaluating physician. This provides clearer insight into expert assessments without requiring manual review of the full report.
